For those who are replacing lost or stolen passports, they need to accomplish Form DS-64. They need to communicate in this form the circumstances regarding the loss of the document. Form DS-64 and Form DS-11 are both US government forms that can be obtained at the website of the State Department.

When the paperwork has been completed, applicants have to make a personal visit to the nearest acceptance center. They should carry with them some form of identification. When asked, they can show a driver's license, a government issued ID, or a military ID. They will get an interview and their documentation will be looked at. After all things are satisfied, the applicants will next pay for the application. It should be noted that there are two types of applications. One is a regular application that takes four to six weeks to accomplish. There is an expedited application where the process takes less than 21 days.
